Breast augmentation with prostheses is an operation technically known as augmentation mammoplasty. It involves placing round or anatomical implants, appropriate to the patient’s physiognomy. Our plastic surgeons at Clínica Sanza, with more than 30 years dedicated exclusively to plastic surgery, advise the patient on what would be best in his or her case, taking into account that the last word is the patient’s.
This operation can be performed in different ways depending on the type of prosthesis we place, where exactly it is placed and the size chosen by the patient. This procedure involves changing the volume and shape of the breasts.

Breast Lipofilling ?
Before proceeding, it is important to understand what lipofilling is!
I advise you to read
this article on our website
where it is explained in detail. It is important …
If you have a good understanding of what lipofilling is, continue on to this section of the website.
There are several ways to increase the size of the breasts, and in Clínica Sanza Barcelona we give you this possibility that more and more people ask us for, it is called breast lipofilling. It is an alternative to putting breast prosthesis.
It consists of a small liposuction of an area where the patient has excess fat (usually the abdominal area or legs), we process it to purify it and then infiltrate it into the breasts.
This technique has certain limits in terms of the size of the breasts we can increase and the shape we can give them. It is not as flexible and does not have as many possibilities as breast augmentation with prostheses.

SURGERY PROCEDURE
Breast augmentation with prosthesis
The physician will perform a pre-assessment of each case to ensure that the final result meets the patient’s wishes and needs. In this phase, the specialist will assess the volume, breast projection, skin quality and physical constitution, among other factors. Depending on the type of breast desired, we will advise round or drop-shaped prostheses with a larger or smaller base and projection.
The doctor will then advise on the access site and the position of the prosthesis prior to surgery. Implants can be placed via submammary, abdominal, transareolar, axillary or periareolar, which is the most common.
The prostheses are placed in three areas: under the gland, under the muscle fascia or under the pectoralis muscle. The volume of the existing breast prior to surgery will determine the location. Generally, they are usually placed under the muscle as the esthetic and functional results are more satisfactory.
Finally, the incision is sutured and drains are placed, which must be worn for 24 hours.
We advise you to provide a photo of the type of breast you wish to have in order to specify more precisely the implant to be placed during the operation and in which anatomical plane.
Breast augmentation with own fat
We must assess the patient. See your augmentation desires, i.e., the size you want to achieve and provided we have a fat-giving area to achieve it. Once you are ready for the procedure, it will be performed in the hospital and will not leave scars since the whole process is done with fine liposuction cannulas.
DURATION OF THE OPERATION
- Breast augmentation with prosthesis: 1 – 2 operating hours
- Breast augmentation with own fat: 3 to 4 hours of surgery
PROTOCOL
Preoperative
- Evaluation of the area to determine the adequacy of the treatment.
- Follow specific guidelines determined by the doctor
- CBC and tests on cardiac and respiratory status
- Ultrasound and/or mammography
Anesthesia
- Local and sedation
Hospitalization
- Ambulatory or 24 h admission
Postoperative breast augmentation with prosthesis
- Medication if required
- Drainage and compressive bandage from 24 to 48 h after surgery.
- Once the drainage and bandage have been removed, special brace
- Mandatory revisions a few days, weeks and months after the operation, according to prescription
Postoperative breast augmentation with own fat
- Taking prescribed medication
- No drainage required
- No breast bandage is required, but a compression garment is required in the area where the fatty tissue is removed.
- You can wear the bra of your choice loosely. The fatty tissue is made up of living cells that cannot be massaged so that they can become incorporated and live well in their new place.
- The same as any graft: skin, muscle, cartilage, bone, etc. It is important to take care of it and not to touch it until 15 days after the graft has taken root and survived. Otherwise it may be reabsorbed
- Mandatory postoperative revisions to control the evolution and to have a good result, without complications.
Convalescence from breast augmentation with prosthesis
- 3 to 4 days
- Avoid carrying weights and sudden movements for 10 to 20 days.
- Wearing a sports bra and comfortable, loose-fitting clothing for the first few days.
- No sports or other strenuous activities for at least one month.
Convalescence from breast augmentation with own fat
- 2 or 3 days of rest for the liposuction area.
- Avoid stress and shocks to the breasts for 15 days.
- Sleep on your back or side, never on your stomach until 15 to 20 days after surgery.
- You can wear any loose bra you want or not even wear it at all.
- No sport until 20-30 days
RESULTS OF THE OPERATION
- Desired breast augmentation
- In the case of breast augmentation with own fat, it improves the body silhouette of the area where the fatty tissue has been extracted.
- Breast repositioning
- Breast balance and symmetry
- Natural and perfect appearance and consistency of the operated breasts.
- Improvement of the patient’s psychological profile and self-esteem.
DEGREE OF DIFFICULTY
- Surgery with medium difficulty
LEVEL OF RISK
- Low risk operation
- Risks inherent to the operation: capsular contracture, implant rupture, implant rotation, rejection, etc.
- Risks of any operation: hemorrhage, infection, impaired healing

Frequently asked questions about breast augmentation with prosthesis surgery
What are the most common reasons for breast augmentation surgery with prostheses?
Larger breasts are a natural desire among women with small breasts, breasts significantly different in shape and volume, and even in women who have small, sagging breasts due to breastfeeding or age. Breast augmentation restores the proportions of the trunk to the waist and improves self-perception and self-esteem.
What are the best prostheses?
This is a difficult question to answer. Is there such a thing as the best car, the best handbag, the best watch?
In Europe there are only EEC (European Economic Community) marked prostheses, and all of them have very rigorous quality controls. that is, they are all equally good.
The only difference between them is the guarantees:
- From 10 years old
- 20 years old
- Lifetime
At Clínica Sanza we only use or recommend the lifetime warranty.
Are smooth or rough prostheses better?
Until 1 or 2 years ago, rough prostheses were used, but now there has been a change since it was discovered that they can cause a type of lymphoma (LACG).
For this reason, at Clínica Sanza we reinsert smooth prostheses but with the same guarantees of contracture and/or breakage for life.
We believe that we do not want risks for our patients even if they are unlikely. We seek maximum security.
Who are candidates for breast augmentation surgery?
All those women who have reached the fullness of breast development and are not satisfied with the size of their breasts because they consider them small, women who dislike the way bras fit because of the shape of their breasts, women whose breasts after periods of breastfeeding become flaccid and sagging, women with sagging breasts due to an accentuated weight loss, women with breast asymmetries (one breast different from the other) and women who require breast reconstruction because they have had lumps.
From what age can breast implants be inserted?
Augmentation mammoplasty can be performed as soon as a person has completed breast development. Some women have already completed this development by the age of 17.
What is a breast implant?
It is an element used to increase breast volume, the most popular and most used implants worldwide are silicone implants, a silica derivative, which can be placed retropectorally (behind the pectoral muscle) or prepectorally (in front of the pectoral muscle).
What is the difference between a round or anatomical prosthesis?
As their names indicate, they are prostheses with different shapes, the round prosthesis is circular in shape where the point of greatest projection is the center of the prosthesis while the anatomical prosthesis is a pyriform prosthesis where the point of greatest projection is towards the lower quadrants. The projections, measurements and shapes are points to be specified in the medical visit where a team of specialists will be waiting for you at Clínica Sanza.
How is the procedure performed?
In Clinica Sanza we perform the procedure in the operating room under local anesthesia and sedation, it is a completely painless procedure where the patient is asleep and does not notice any painful stimulus.
Does it require hospital admission?
The procedure usually does not require hospital admission, it is on an outpatient basis, unless the patient requires it.
What is the postoperative period like?
The postoperative period is slightly painful and tolerable, where the patient will wear a compressive bandage and drains for the first 24 hours, which will later be removed. Then we will place a special postoperative bra and in many cases an elastic band around the thorax. Recovery is gradually progressive with noticeable improvement after 72 hours, when the patient can resume normal activities of little physical effort.
How long do I need to be off work?
Recovery for work activities averages between 10 and 14 days….. However, each case is individualized according to the physical effort made in your work; in some cases less days of rest are required and in others more.
Are the scars very visible?
Surgical approaches leave scars, which at Clínica Sanza we do our best to make imperceptible. The surgery can be performed depending on each case by means of approaches in the axilla, in the areola or in the submammary fold. Generally, following the indications and postoperative recommendations, whatever the approach used, the scars are hardly visible and practically imperceptible some time after the surgery.
Will I be able to breastfeed my child (lactation) after breast augmentation?
There is no inconvenience with breastfeeding, after the recovery and the tissues are healed, the tissue restitution is complete and the patient can breastfeed normally.
Will I lose sensitivity of the areola and nipples?
Sensitivity is not affected in most cases, however in some cases in the first months after surgery it is the opposite, there may be hypersensitivity; in other less frequent cases there may be a decrease in sensitivity, but in both cases after a short time it recovers completely.
