The breast reduction surgery or reduction mammoplasty at Clínica Sanza in Barcelona, Spain, consists of mammary hypertrophy correction, a pathology caused by the exaggerated increase of the mammary volume. This increase may take place at puberty. It is known as virginal breast hypertrophy and it is due to a breast hypersensitivity to female hormones. In these cases the breast usually presents a homogenous, turgid, strong and elastic, but large and uncomfortable appearance.

As people get older, the mammary hypertrophy is usually due to the progressive increase of the fat tissue which conforms the breast and may or may not accompany the overdevelopment of the glandular tissue. The breast is large, less turgid, less elastic, with poor internal vascularity and a marked superficial venous network.

The increase of volume altogether with the characteristics of the skin can produce stretch marks, which are nothing more than scars caused by the rupture of the skin, being unable to contain the expansion of the tissue.

The considerable increase of breast volume usually affects the quality of life of many patients, conditioning their way of dressing, sport practice, social development and also affecting individual self-image. Besides, from the aesthetic considerations already mentioned, there are also functional issues. Overdeveloped breasts can produce alterations in the bipedestation dynamic causing imbalance and poor support for the dorsal spine. There are other problems,among other pathologies,  such as, skin alterations at the level of the submammary groove and  shoulder pain caused by the bra.

PROCEDURE

There are different techniques for the resolution of this pathology, and all of them aim to elevate the areola-nipple complex; reduce breast size; achieve volume, shape and symmetry as perfect as possible, and all with a minimum of residual scars possible. Dr. Sanza gives great importance to the scars and is therefore well known for his minimal scar techniques and better results.

The choice of one technique or another, will depend on the characteristics and volume of the breasts. Among the mammary characteristics that most determine it we find the height of the nipple, the width of the mammary base, the breast extension and the intermammary distance.

Reduction and elevation surgery are similar and they share common steps. The majority of patients who have breast hypertrophy problems usually also have some degree of breast ptosis (dropping breast), so both problems must be corrected in the same procedure; that is, to lower and raise the breasts at the same time.

During the procedure, we always try to minimize the scars, which is why we never, or hardly ever, leave any horizontal scar (the famous inverted “T” or anchor), a technique that despite the cicatricial stigmas, is still being done too often nowadays (unfortunately).

The scars usually trend to look very thin and barely visible in most cases, and on the few occasions when there is a scar that is too visible, corrective procedures can always be carried out; either by a small retouch of these scars in order to make them almost imperceptible, or even tattooing them.

Currently we have a device that we are already use in the unit of mammary surgery of Clinica Sanza, for the disappearance of scars or at least to obtain its maximum attenuation, even in cases of mammary striae or from other corporal zones. If they do not disappear completely, we ensure that they will be much more imperceptible and that they will improve in an incredible way.

DURATION OF THE SURGERY

  • From 2 to 3 hours.

PROTOCOL

Pre-surgery

  • Evaluation of the area to determine if the treatment is suitable.
  • Following the correct guidelines that are set by the doctor.
  • Blood tests and general testing (cardiac and respiratory diagnostics).
  • Ultrasound scan and/or mammography.

Anesthesia

  • Local and sedation, and in some cases, general.

Hospitalization

  • Ambulatory or 24h hospitalization.

Post-surgery

  • Medication if required.
  • Drainage and compression bandage from 24h to 48h.
  • Special plasters for 4 or 5 days.
  • Once drainage and bandages are removed, use of a special bra.
  • Mandatory revisions after a few days, a few weeks and a few months, according to prescription.

Convalescence

  • From 4 to 5 days.
  • Avoid picking up weights and making abrupt movements from 10 to 20 days.
  • Use of sports bra, and comfortable and loose clothing the first few days.
  • Do not play sports or activities that require effort for at least a month

What is a breast reduction?

Breast reduction, technically known as Reduction Mammoplasty, is the surgical procedure aimed at reducing the gland, fat tissue and the skin that surrounds it, forming smaller, higher, tighter and more harmonized breasts.

Who are the candidates for breast reduction?

Most patients have surgery because the large breast volume restricts their normal activities. Women with large breasts may experience a number of problems related to breast weight and size, such as back, neck and shoulder pain and skin irritation. All women who have large breasts and/or have any of these symptoms are a good candidate for surgery. Also in asymmetrical breasts or women that cause them a psychological problem when undressing.

How’s the procedure?

The techniques are very varied, in Clinica Sanza we adjust it to the best result for each patient. Although all of them seek a higher position of the areola, a narrowing of the base and above all that the form is natural. The surgery consists in the removal of skin, gland and fat, moving the mammary areola to its new position. The scar in this surgery is generally around the areola and a vertical scar and in some cases requires a horizontal scar that with special care may be little visible.

How long does the surgery take?

About 2 or 3 hours.

Will I lose the sensitivity of the areola and nipples?

Usually it is not lost and if it happens the sensitivity recovers in a few months. This will depend fundamentally on the type of breast reduction, that is to say, on the volume prior to the surgery since in situations of gigantomastia, the mammary nerves are so distended that they will not be able to recover. In the first few weeks it will be normal to have a corking sensation, but it is normal for you to recover your sensitivity in the months following the surgery and remain as before the intervention.

Are the results permanent?

The results are permanent but not for life. All breasts tend to droop as a result of gravity. Since the volume of your breasts after breast reduction will be less, they will fall more slowly, but you should keep in mind that it is the quality of your skin that will fundamentally determine the rate at which your breasts fall.

How’s the post-op?

The postoperative period is slightly painful and tolerable. In the first 24 hours the patient will wear a compressive bandage and drains, which will later be removed. Then we’ll put on a special post-op bra. The recovery is gradually progressive with noticeable improvement after 72 hours where the patient can be incorporated into regular activities of little physical effort.

Can I breastfeed after breast reduction?

Of course you can. If the patient is young and wishes to breastfeed, we will remove more fat and less breast tissue. If, on the other hand, you do not want to breastfeed or you have already had children, we advise you to remove more glandular tissue so that your breasts cannot grow during menopause.

Can it be done without scarring?

There is no way to reduce breast size other than by surgery. Now, depending on the previous size and the desired size we can do three techniques: -Periareolar: scar only on areolas -Vertical: Scar on areolas and up to submammary furrow. T inverted: as the vertical but a scar more in submammary furrow in the form of anchor. In Sanza Clinic we make 90% of reductions only with periareolar or vertical, and scars are barely noticeable after 1 year of surgery.

Does a breast reduction look nice?

Of course I do. It is one of the most gratifying aesthetic surgeries for several reasons: more beautiful breasts, more youthful, more comfortable and more elegant and discreet.
